Reduced folate cofactor (5-FU potentiator)

Leucovorin (calcium folinate)

folinic acidcalcium folinate

How it works

Stabilises the ternary complex of FdUMP with thymidylate synthase, potentiating 5-FU cytotoxicity. Given immediately before 5-FU in FOLFIRINOX / mFFX.

Biochemistry matters: leucovorin must precede 5-FU by a sufficient interval for ternary-complex formation. Verify dose per institutional protocol (eviQ 1512 uses 50 mg; original FFX uses 400 mg/m²).

Dosing

Typical doses

  • 400 mg/m² IV over 2 h (standard FFX); eviQ 1512 reduces to 50 mg

Schedule

  • D1 of 14-day mFFX cycle

Side Effects

  • No direct toxicity at usual doses; occasional rash / allergic reaction
